If there is a “thick or low position on top”, as per description, and white plays ‘3’ (P16), then what happens? Position:18120 looks ugly…

Good question. It is an ugly position and also a common technique in a variety of situations.

I meant to remove the “thick or low” comment last night. It’s removed now and I added some variations to https://online-go.com/joseki/18120 . We will have to work it some. There is a lot of overlap with moves 5, 1, and 2 in this position.
